    Prise en Charge des Avortements: Experiences de Cinq Structures Peripheriques dans un Pays en Voie de Developpement

    Objectif : DĂ©terminer le profil sociodĂ©mographique des patientes, dĂ©crire les aspects thĂ©rapeutiques des avortements et identifier les facteurs associĂ©s aux avortements. MatĂ©riels et mĂ©thodes : Il s’agissait d’une Ă©tude transversale, multicentrique, Ă  visĂ©e descriptive et analytique rĂ©alisĂ©e pendant 12 mois du 28 fĂ©vrier 2022 au 31 janvier 2023. L’étude a eu pour cadre, (05) maternitĂ©s pĂ©riphĂ©riques de BOUAKE assurant la prise en charge des avortements. Étaient incluses toutes les patientes ayant bĂ©nĂ©ficiĂ© dans l’un des centres de notre Ă©tude d’une prise en charge suite Ă  un avortement complet ou incomplet, provoquĂ© ou spontanĂ©. Les donnĂ©es ont Ă©tĂ© recueillis et analysĂ©s Ă  l’aide du logiciel Epi Info version 7. Le seuil de significativitĂ© des tests statistiques Ă©tait fixĂ© pour une valeur de p≤ 0,05. RĂ©sultats : La prĂ©valence des avortements Ă©tait de 5% avec (avortements provoquĂ©s 60% et avortements spontanĂ©s 40%). L’âge moyen Ă©tait de 27 ± 7 ans. Les patientes avaient un niveau d’étude primaire ou non scolarisĂ©es dans 48% des cas et non salariĂ©es dans 93% des cas. Elles Ă©taient cĂ©libataires ou vivant en concubinage dans 79% des cas. Elles Ă©taient venues d’elles-mĂŞmes dans 90,2% des cas avec pour principal motif d’admission les mĂ©trorragies (89,9%). Les motifs des avortements provoquĂ©s Ă©taient la grossesse non planifiĂ©e (69%), le refus du partenaire (26%). Les patientes avaient prĂ©sentĂ© des complications dans 30% des cas. L’évacuation utĂ©rine a nĂ©cessitĂ© l’aspiration manuelle intra utĂ©rine (45%) suivi de l’ocytocine (32%). L’âge ≤ 24 ans, le statut non salariĂ©, le statut non mariĂ©, le faible niveau d’étude et la multiparitĂ© Ă©taient significativement associĂ©s Ă  l’avortement provoquĂ©. Conclusion : La prĂ©valence des avortements demeure Ă©levĂ©e Ă  BouakĂ©. Il serait bĂ©nĂ©fique de faire la prĂ©vention par la planification familiale et amĂ©liorer la prise en charge des avortements. Objective: To determine the socio-demographic profile of patients, describe the therapeutic aspects of abortions and identify the factors associated with abortions. Material and methods: This was a cross-sectional, multicentre, descriptive and analytical study conducted over a 12-month period from 28 February 2022 to 31 January 2023. The study was carried out in (05) maternity units on the outskirts of BOUAKE that provide abortion care. All patients who had received treatment for a complete or incomplete, induced or spontaneous abortion at one of the centres in our study were included. Data were collected and analysed using Epi Info version 7 software. The significance threshold for statistical tests was set at p≤ 0.05. Results: The prevalence of abortions was 5% (induced abortions 60% and spontaneous abortions 40%). The mean age was 27 ± 7 years. In 48% of cases, the patients had a primary education or were not at school and in 93% of cases they were not employed. They were single or cohabiting in 79% of cases. They were self-referred in 90.2% of cases, with metrorrhagia the main reason for admission (89.9%). The reasons for induced abortion were unplanned pregnancy (69%) and partner refusal (26%). Complications occurred in 30% of cases. Uterine evacuation required manual intrauterine suction (45%), followed by oxytocin (32%). Age ≤ 24 years, unpaid status, unmarried status, low level of education and multiparity were significantly associated with induced abortion. Conclusion: The prevalence of abortions remains high in BouakĂ©. It would be beneficial to promote prevention through family planning and to improve the management of abortions
